WHAT TO BRING

Please make sure you bring your mammogram films and reports (we do not accept CD disks of mammograms). Ultrasound reports, biopsy reports, pathology reports. All of these from the past 3 years are required so Dr. Kemp can evaluate your history properly.

All medications, please put all of your medications in a bag and bring them in.

If you have an insurance that requires a referral, please contact your doctor that is sending you to us. You will need to bring a computer generated referral to your appointment or have it faxed for you. Failure to obtain a referral may result in charges to you. Thank you.

Please bring a copy of your insurance card, both front and back and your driver license.

If you have new insurance, please call ahead of time and inform us so we can properly verify your benefits before your arrival.

Anytime your address, phone, insurance changes, kindly call us with the updates.

Please note that "No Show" or "Cancelled" appointments in this office are subject to a $50.00 cancellation fee if you have not notified us 48 hours before your scheduled appointment. Please understand that we are very diligent about scheduling all of our patients in a reasonable amount of time. At the time of your visit, Dr. Kemp may do a biopsy on the same day if needed. Our appointments are meant to be kept on time but may possibly run late. You will get Dr. Kemp’s undivided attention and thorough exam on your visit. Thank you in advance for your patience and understanding.
