If you have an insurance that requires a referral, please contact your doctor that is sending you to us. You will need to bring a computer generated referral to your appointment or have it faxed for you. Failure to obtain a referral may result in charges to you. Thank you.

If you have new insurance, please call ahead of time and inform us so we can properly verify your benefits before your arrival.

Please note that "No Show" or "Cancelled" appointments in this office are subject to a $50.00 cancellation fee if you have not notified us 48 hours before your scheduled appointment. Please understand that we are very diligent about scheduling all of our patients in a reasonable amount of time. At the time of your visit, Dr. Kemp may do a biopsy on the same day if needed. Our appointments are meant to be kept on time but may possibly run late.
